If you are interested in pursuing Opportunity Homes, Inc. services, contact the Administrative office at 563-382-8140 for referral information. Our referral process begins with an Application for Services as well as a social history. This information is presented to the Admissions Committee. The committee will determine the agency's ability to provide quality services to the applicant based upon Opportunity Homes, Inc. policies. After completing the required information, applicants are encouraged to speak with their case manager or CPC (Central Point of Coordination) to discuss funding options available for the person requesting services. To inquire about funding, speak with the Central Point of Coordination for your county of legal settlement. Upon the decision to receive residential services, the applicant will be contacted by the Administrative office for further information as well as to complete the admissions paperwork for provision of service.
