Opportunity Homes, Inc. provides individuals the opportunity to participate in their home of choice in order to attain the highest degree of independence. The individual consumers drive the services based upon the philosophy of choice and responsibility regarding the development and enhancement of independent living skills. These services are delivered according to each individual's wants, needs and desires. A strong emphasis is placed upon community integration through establishing natural supports for the individual served. Services considered necessary and appropriate for the individual are determined through an interdisciplinary team consisting of the individual, family members, guardians, case management (if applicable), service providers and others chosen by the individual. HCBS-Supported Community Living Home and Community Based Mental Retardation Waiver (HCBS-MR) provides individualized supports to eligible persons in their own homes or communities who would otherwise require care in an institution. The individual must have a diagnosis of mental retardation or a diagnosis of a mental disability equivalent to mental retardation as determined by a psychologist or psychiatrist. This service service is designed to assist the individual with daily living needs. Assistance may include, but is not limited to: personal and home skills, community skills, personal needs, transportation and treatment services. MR Waiver services are individualized to meet the needs of each person. However, decisions regarding what services are appropriate are based on the individual's needs as determined by the individual and their interdisciplinary team. Services are habilitative in nature and provide for development of skills towards individual achievement.
Family Support Services - Respite This program is designed to provide temporary care to children or adults to sustain the family or other primary caregiver by providing time-limited and temporary relief from the ongoing responsibility of care. These services may be provided on either a planned or emergency relief basis in the person's home. The purpose of respite care it to enable the person to remain in their current living situation. Respite services cannot be provided to children residing in the family, guardian or usual caregiver's home during the hours in which the usual caregiver is employed. Chapter 24 -Supported Community Living Individuals who experience mental retardation or are dually diagnosed with mental retardation and developmental disability may be eligible for these services. This program provides training to persons who have a wide variety of disabilities and are capable of living semi-independently in a community setting. This may include the provision of assistance in all areas of life skills including budgeting, cooking and home management. Service provision is similar to the provision of HCBS-Supported Community Living, however is funded and regulated by different regulatory entities. Chapter 24-SCL services are provided in hour units. Chapter 24-SCL services provided by Opportunity Homes, Inc. are habilitative in nature, promotion development of skills and support services necessary for individual achievement. These services are provided within each individual's home and community. |
